Participant Waiver and Release of Liability

EXHIBIT A: PARTICIPANT WAIVER AND RELEASE OF LIABILITY

I, [Participant Name or Parent/Guardian Name], in consideration of participating in the Virtual Reality activities ("Activity") provided by Jade Pro Entertainment, LLC, acknowledge and agree as follows:

  1. ACKNOWLEDGMENT OF RISKS I understand that the Activity involves inherent risks, including but not limited to motion sickness, disorientation, loss of balance, collisions with objects or persons, eye strain, seizures, or aggravation of pre-existing medical conditions. These risks may result in serious injury, psychological harm, or, in rare cases, death. I voluntarily assume all such risks.

  2. HEALTH AND SAFETY I certify that I (or the minor Participant) am in good health and have no medical conditions that preclude participation in the Activity. I agree to follow all safety instructions provided by Jade Pro Entertainment, LLC, including wearing protective gear and using equipment as directed.

  3. RELEASE OF LIABILITY I, for myself, my heirs, executors, administrators, and assigns, hereby waive, release, and forever discharge Jade Pro Entertainment, LLC, its employees, agents, affiliates, and successors from any and all claims, damages, losses, or liabilities arising from my participation in the Activity, including but not limited to personal injury, property damage, or emotional distress, except those caused by the gross negligence or willful misconduct of Jade Pro Entertainment, LLC.

  4. EQUIPMENT RESPONSIBILITY I agree to use the VR equipment with care and not to damage, modify, or remove it from the designated area. I acknowledge that I may be held liable for repair or replacement costs if I cause damage to the equipment through negligence, recklessness, or willful misconduct.

  5. PHOTO/VIDEO RELEASE (Optional) I grant Jade Pro Entertainment, LLC permission to use photographs or videos of my participation in the Activity for promotional purposes, unless I opt out in writing.

  6. PARENT/GUARDIAN CONSENT (If Applicable) If signing on behalf of a minor, I certify that I am the parent or legal guardian of [Minor’s Name], and I consent to their participation in the Activity under the terms of this Waiver.

I HAVE READ AND UNDERSTAND THIS WAIVER AND RELEASE OF LIABILITY. I AM SIGNING IT FREELY AND VOLUNTARILY, AND I UNDERSTAND THAT IT IS A LEGALLY BINDING CONTRACT.
